What is Nursing Care Plan?
Nursing Care PlanA nursing care plan is a written document that outlines the care to be provided to an individual patient. It includes goals and objectives, interventions, and evaluation of outcomes. The purpose of creating a nursing care plan is to make sure that patients receive safe and effective care based on their needs. Nursing care plans are developed by nurses in collaboration with other healthcare providers, such as physicians or social workers, who may be involved in the patient’s treatment. Goals & Objectives: The goal of a nursing care plan is for the patient to achieve maximum health potential given their current condition and any limitations they have due to illness or injury. Objectives are specific steps taken towards reaching this goal. For example, if the goal is to improve mobility then objectives might include exercises or physical therapy sessions at certain intervals throughout the day; if the goal is pain management then objectives may involve medications or relaxation techniques. Interventions: Interventions refer to treatments and therapies used by nurses and other healthcare professionals in order to reach desired goals outlined in the nursing care plan. These can range from medication administration, wound dressing changes, teaching patients self-care practices such as how to use crutches correctly, providing emotional support, dietary adjustments, physiotherapy sessions etc.. Evaluation: Evaluation involves measuring progress made towards achieving desired outcomes as stated within the documents created during development of nursing care plan (such as improvement in mobility). This also enables nurses/healthcare practitioners identify areas that require further attention and adjust interventions accordingly according to changing needs of individuals under their charge – thus ensuring continuity of quality standards when it comes delivering personalized services tailored specifically for each person receiving said services/treatment.
