You are a Social Care worker and a service user, Hannah, tells you that she is unhappy taking her new medication. She thinks she does not need it and so she is throwing it away. You know from her care plan that Hannah does need to take the medication regularly and gets confused. Hannah begs you to keep this confidential and not to tell anyone especially her daughter, who she sees regularly, as her daughter will be very angry. Bi.

This will mean that all information related to them will be kept in strict confidence for use only by the team of care providers. Indiscretion is an action in which you inadvertently share confidential information. There is no bad intent associated with an indiscretion. Informed consent is when a resident or client acknowledges and allows the release of information to other parties. This permission is given by filling out a legal consent form, which becomes part of the resident or client’s permanent record. Private is defined as not available for public viewing or knowledge. may need to be broken as keeping such information to yourself causes you concern as it could result in harm to the person you are supporting, to someone else or to yourself. It is a basic requirement of my job role to communicate with individuals and their families, other members of staff on a daily basis. Communicating with other staff members ensures effective team working and continuity of care. It also ensures any health and safety issues are recognised and reported. All carers attend hand over at the beginning of each shift and also complete communication books after attending an individual, thereby keeping other staff informed and aware of current situations within the workplace. Individuals communicate with carers to express their needs and preferences and to ensure they are met.

As a carer I would discuss the options and choices available to the individual to allow them an informed choice regards their care. Aii. Describe two ways how

communication can effect relationships in an adult social care setting between individuals using the service, their carers, colleagues and other practitioners. Effective communication is important as it helps to pass clear, concise, informative and accurate information in order to decrease and eliminate the possibility of mistakes and risks to the people I am working with.
