The aim of this research is to analyze the health issues in Sentinel City, a virtual town that has mostly white residents but also some diversity. Despite being impacted by multiple factors, the city confronts notable community health challenges. The goal is to emphasize both the favorable and unfavorable influences on the city, focusing specifically on the main community health problem observed during the visit – a rise in certain sexually transmitted diseases.

There is concern about the rise of STDs in the community, particularly among the 15-24 age group. The city has experienced a significant increase in cases of Gonorrhea and Chlamydia, with these two diseases having the highest percentage of cases. It should be noted that nationally there has been a 2.8% increase for Gonorrhea and a 5.1% increase for Chlamydia according to CDC's report in 2015. This emphasizes the i

mportance of addressing this issue specifically in this city. To assess the needs and risks, both bus and walking tours were conducted.

I found relevant information at various locations in town. The demographics assessment tool provided a great starting point for identifying the race, sex, and economic status of this community. The neighborhood safety inventory section helped me identify existing resources and potential needs in this city. The windshield survey was the most valuable section as it allowed me to physically go into the city, observe and understand the problems that it was facing. Lastly, the population health scavenger hunt helped me discover local services and programs that are currently available.

In 2012, Sentinel city had a total population of 634,265. The age distribution is as follows: individuals aged 19-64 make up 60% of the population,

those under five years old account for 7.4%, individuals under eighteen years old make up 21.7%, and those sixty-five or older comprise 10.5%. In terms of ethnicity, the majority (52.7%) are Caucasian, followed by Hispanics at 31.5%, African Americans at 10.4%, and Asians, American Indians, Hawaiians, and bi-radials collectively making up approximately 9% of the population in this city. Additionally, there is an estimated poverty rate of 18.9% among the residents.

The median income for the residents of Sentinel City is $49,091. The city has four distinct neighborhoods: Nightingale Square, Acer Tech Center, Casper Park District, and Industrial Heights.

Looking at the demographics of each neighborhood reveals that the majority of residents reside in the Casper Park District with a population of 352,643. In this district, families have a median income of $80,134 and an uninsured rate of 22.7%.

The following neighborhood is Acer Tech Center with a population of 168,390.

The neighborhood in question has a low uninsured rate of 1.5% and residents have a median income of $166,300. Nightingale Square, on the other hand, boasts a higher median income of $269,550 but has a smaller population of 103,974. Industrial Heights has the smallest population with only 38,855 residents and an alarmingly low median household income of $24,672. Furthermore, this neighborhood suffers from a high non-insured rate of 37.5% compared to others.

According to the Neighborhood/Community Safety Inventory report, numerous safety hazards were identified including an increase in food safety violations from the previous year. The prevalence of pollution is evident as cough remains the leading cause for emergency room visits while air pollution affects various parts of the city. Insufficient maintenance has led to deterioration in many buildings

within Industrial Heights and Casper Park district neighborhoods. Stray dogs and cats are frequently spotted throughout the city which may contribute to the observed rise in dog bites specifically in Industrial Heights.

There are several areas of ongoing construction in the city that could pose potential safety concerns if not properly supervised. Additionally, there is trash littered along many store fronts, which is detrimental to the safety of citizens and animals as it promotes the formation of disease-causing microorganisms. The presence of graffiti on various buildings throughout the city also suggests the existence of gang activity. The predominant drug being used in this city is Marijuana, as indicated by the current arrest rate for this substance, which stands at 23%.

In this town, the arrest rate for controlled substances is 16%, and there is a significant concern about EMS response times. The overall EMS response time in this town is 7.46, which outperforms two of the three comparison cities. However, an increase in V-fib patients has led to a decline in the EMS response time. As for crime rate, the most frequent arrests in this city include warrants, drug charges, and battery. There appears to be a decrease in violent crimes over the past year. Despite this improvement, gang violence remains a persistent issue within the city.

The town of Sentinel City is facing a significant issue concerning gang activity, which includes crimes such as simple assault, robberies, homicide, and aggravated assault. While the number of aggravated assault incidents has decreased in the past year, the overall offenses remain alarmingly high. During my visit to Sentinel City, I thoroughly examined the town by using bus transportation and frequently

exploring on foot. This approach enabled me to make several observations about various aspects of the town. One notable discrepancy that caught my attention was the difference in building conditions between affluent areas like Nightingale Square and Acer Tech Center compared to other neighborhoods.

Local cafes in various neighborhoods are experiencing a rise in citizens socializing. These areas have relatively low graffiti and trash levels, indicating better maintenance compared to lower income neighborhoods. The presence of apartment buildings and condos is noticeable, while suburban areas with yards and houses are absent. The majority of the neighborhoods comprise concrete and brick structures; however, there are small grassy sections at the school yard and one apartment complex within Casper Park District.

The presence of does not disguise the fact that this location is poorly maintained. The building is in disrepair and the grass is overgrown, indicating a lack of community pride. This is especially evident in the Industrial Heights and Casper Park District neighborhoods. In this densely populated city, buildings and apartments are closely packed together, leaving no space for yards. The deteriorating buildings and environment accentuate the divisions within the city, which can be attributed to economic factors.

During my visit to the city, I noticed that most children were playing at school while there were few common areas where adults were seen socializing. However, I did observe a local cafe in the Acer Tech Center where people seemed to gather and socialize. Additionally, Nightingale Square seemed to be another hub of socialization, particularly at the tennis court/park area. Here, I saw musicians and jugglers performing and attracting a crowd. These social areas were open to everyone, but

it seemed that there were more business people present.

In the other neighborhoods, there were no significant gatherings of people. Couples were observed walking the streets or sitting on scattered park benches. During my visit, I noticed three main modes of transportation: cars, buses, bicycles, and pedestrians. In Nightingale Square and Acer Tech Center, there were numerous cars in the community, and a significant number of pedestrians were also present. In Casper Park District and Industrial Heights, cars were present but in lesser quantities.

These neighborhoods have experienced a decrease in car quality and condition. People can be observed waiting for buses at bus stops, cycling, or walking around. The town has only one centrally located gas station and is lacking significant highways or railroads. Construction is ongoing in some areas, with certain roads and alleyways requiring resurfacing. Fortunately, the city's department of transportation has set aside funds to tackle these problems. Furthermore, this town houses a range of medical professionals including doctors and dentists who provide both inpatient and outpatient services.

The majority of the facilities are located in the Acer Tech Center, but there is also a health clinic in the Industrial Heights neighborhood. Both areas appear to be bustling with activity, as patients and staff are constantly coming and going from the buildings. In addition, there are a few parks in the city. One park is situated near the school, where many children were happily playing during my visit. Another park, Nightingale Square, had both basketball and tennis courts being utilized when I visited. Furthermore, the city offers various services for parks and recreation such as an after-school program, swimming lessons, a summer program for

kids, and more.

Other service agencies in the town include WIC, SNAP, and Adult and Children Protection Services. Adoption assistance is increasing. Daycare centers are available in various settings such as workplaces, homes, and facilities, with most being for profit. The city offers various senior services. Numerous grocery stores and markets are located in the town. In Nightingale square, Joes Grocery is the primary grocery store, which appears to be bustling with numerous customers coming and going. Acer Tech Center also has a farmers market.

Both neighborhoods have convenience stores, as well as many restaurants and food vendors spread throughout the city. Nightingale Square and Acer Tech Center are home to several shopping centers and stores. Furthermore, the local church runs a soup kitchen for the homeless population. When I visited, the town was lively with people. They were observed congregating outside at local cafes and on different benches around town, while also strolling along sidewalks by stores and shops.

Various bike riders are in the Acer Tech Center area, while others are observed waiting on buses at the bus stops across the city. Police officers are present in all neighborhoods, patrolling streets and driving vehicles. Protestors congregate in front of city hall. Alongside them, a crossing guard assists several children on their way to school. During my visit, numerous people were queued up for the soup kitchen. Furthermore, individuals can be seen walking their dogs, and stray animals also roam throughout the city.

This town is showing signs of vibrancy and improvement. Construction is taking place in the Industrial Heights neighborhood to enhance living conditions. The community is active, with people frequenting local vendors and shops. Cultural expression

is evident with the presence of an art gallery. The Acer Tech Center area boasts a historic city hotel. The city's parks and recreational facilities are well-utilized by residents. Meanwhile, protesters in the city hall area are fighting against taxes that burden the less fortunate.

Despite some problems like trash, stray animals, and abandoned buildings in certain areas of the city, it also boasts diversity and a diverse selection of restaurants. While the majority of individuals I observed were white, there was also a presence of Hispanic and black individuals during my visit. Although most signs are in English, you can find Spanish signs in the Industrial Heights neighborhood.

During my visit, I observed a solitary church that serves as a place of worship for people from all faiths. The church also operates a daily soup kitchen. The community faces concerns regarding their well-being, with a noticeable prevalence of respiratory issues and sexually transmitted diseases (STDs). Frequent visits to the emergency room are often due to pain and coughing. General surgery and orthopedic surgeries are the most commonly performed surgical procedures in this area. Additionally, alcohol and tobacco usage is widespread, with around half of the population admitting to using one or both substances. Furthermore, there is a significant presence of individuals with high cholesterol, hypertension (HTN), coronary artery disease (CAD), and diabetes in the city, which is evident from pharmacy data.

The Acer Tech Center, located midway in the city, houses both the Emergency Room and most offices. Additionally, there is a health clinic in the Industrial Heights area. I believe these locations are conveniently accessible, with minimal travel time required. Moreover, this city provides abundant media

resources, including cell phone towers and satellite dishes. Various book stores and magazines are available in certain areas, along with appliance stores. The city is also renowned for its numerous coffee shops, food vendors, cafes, and restaurants scattered throughout town, some may argue excessively so.

Liquor stores and convenience stores selling liquor were also found. Billboards displayed positive messages encouraging vaccination, discouraging texting and driving, and promoting recycling. The Parks and Recreation department offers various activities such as after-school programs, swimming lessons, nutritional courses, gardening courses, city sports leagues, adult fitness courses, and a summer program for children. The Healthcare System-Elder Services provides services like senior transport, government entitlement assistance, meals on wheels, community centers, service animals, elder abuse prevention services, and medical care advocates.

For the elderly, there are multiple options to choose from, including senior apartments, senior living centers, and nursing homes. City Hall-Social Services offers a range of services like police, fire, EMS, social services (WIC and SNAP), water and sanitation services, as well as animal control services. Moreover, the Community Health Center provides various health services such as contraceptive services, pregnancy testing, fertility assistance, basic infertility services, preconception services STD services breast and cervical cancer screenings along with other preventive health screening services.

The Soup Kitchen serves around 40,000 meals per month to homeless individuals and relies on the supplement food program for its main source of food.

In the Casper Park District, there is an Affordable Housing Project that mostly offers two-bedroom apartments. Currently, these apartments have a 45% occupancy rate. There are also studio and one-bedroom apartments available. The average income in this area is $33,000. Some residents have raised concerns about factory

smoke presence, but a recent study shows a decrease in crime rates within the neighborhood.

Based on data analysis, it can be concluded that this town is experiencing growth both in terms of diversity and development initiatives.

Efforts are being made to tackle the recent rise in crime and gang activity, through awareness programs and increased police presence, as shown by the figures.

In the last year, there has been a decline in teenage pregnancy rates, which were previously increasing. Nevertheless, the prevalence of sexually transmitted diseases (STDs) continues to be a major health concern for this community, particularly among individuals aged 15-24.

The city has observed a significant surge in Gonorrhea and Chlamydia diagnoses in the past year, mirroring a nationwide trend (CDC, 2015).

In my opinion, pollution is a significant concern that requires more attention in this community. Additionally, there is a need for construction improvements in this growing city. Moreover, I have observed a notable economic disparity, which is a prevalent issue in many communities.

Concerning Sentinel City, the top three health concerns I have are the high rates of STDs, the negative impact of pollution on air quality, and the excessive number of respiratory diagnoses and complaints. I believe that the latter issue may be linked to the current problem of pollution affecting the city.

The Office of Disease Prevention and Health Promotion (ODPHP) has identified three main health concerns in relation to the Healthy People 2020 goals. One major concern is high STD infection rates, which are a problem not only in Sentinel City but also across the country. To tackle this issue, Healthy People 2020 aims to promote healthy sexual behaviors, enhance community capacity, and improve

access to quality services for preventing STDs and their complications (ODPHP, 2016). Another significant concern is pollution, which poses a challenge on both national and global levels.

Healthy People 2020 aims to promote health for all by promoting a healthy environment, specifically focusing on environmental health goals. One of these goals is improving outdoor air quality (ODPHP, 2016). In Sentinel City, there has been an increase in respiratory diseases diagnosed in the past year. This issue aligns with Healthy People 2020's objective of enhancing respiratory health through education, prevention, detection, and treatment (ODPHP, 2016).
