30+ plans available that include: -Treatment Benefits -Surgery Benefits -Travel Benefits -Additional Benefits Avialible in all four benefit levels

PART 1 -covers 24 tests -payable once per covered person per year PART 2 -covers additional invasive diagnostic test or surgery -payable after abnormal result from one of the covered wellness tests in part 1

-Purchased in $1000 units up to $10k -payable once per covered person by rider -Kids benefit pays 2.5x amount for named insured (no skin cancer/pre-existing cancer condition during waiting period payment)

Payable once per covered person insured by rider (no skin cancer/pre-existing cancer condition during waiting period payment)

Pays $300 per day when covered person is hospitalized for a specified disease IF: -date of diagnosis is after waiting period -date of diagnosis is while poicy is in force -covered person is dconfined to a hospital for treatment of a specified disease -specified disease is not excluded by name or specific description in the rider

Benefits are payable for cancer treatment (incl skin cancer) if: -Date of diagnosis is after waiting period -date of diagnosis is while policy is in force -cancer or treatment is not excluded BY NAME

-Full Time-17.5 hrs/week -meets issue age requirements -Answer NO to internal cancer question on AccHlth application. (if YES to Skin Cancer; will be issued with SKIN CANCER EXCLUSION RIDER -Have never been diagnosed with AIDS / AIDS related condition

-meets issue age req. -Answered NO to Internal Cancer question -No AIDS

-meets dependent child definition -NO to internal cancer question -no AIDS

17-75 premium rates are the same regardless of risk class or age

-Spouse may be listed as primary insured if Employee isn't eligible -Submit AccHlth Appl & Underwriting HIPPA Authorization form with each application -ADDITIONAL FORMS -skin cancer exclusion rider -cancer history form -guide to Health insurance for People with Medicare -Medicare Certification Notice -Medicare Notice - No restriction of # of LEVELS that may7 be offered (employer can limit # of levels) -Cant be sold with critical illness plan that includes cancer -applies to both new & existing accts AND both GROUP & INDIVIDUAL CRITICAL ILLNESS products

NOT allowed -may transfer to CANCER ASSIST from a DIFFERENT POLICY -New application with health questions must be completed -if insured has had cancer within past 5 years cannot transfer to Cancer Assist but family members may

-Not allowed -Insureds can have only one cancer policy at a time

Portable

Employees can keep it for Life as long as premiums are paid
