When is Acapella indicated for use?

-indicated for use in conjunction with a medical need for positive expiratory pressure(PEP) therapy. It may also be used simultaneously with a nebulized aerosol drug delivery -combines positive expiratory pressure with vibratory features -Prevent and reverse atelectasis

What are the initial setting for Acapella?

-If this is the first use of acapella choic, ensure that the frequency adjustment dial is set at the lowest frequency resistance setting which is 1 -Instruct the patient to relax while performing diaphragmatic breathing and inspiring a volume of air larger than normal tidal volume(but not total lung capacity) -Direct patient to exhale to functional residual capacity(FRC) actively, but not forcefully, through device -Adjust dial to increase resistance -The patient should be able to exhale for 3-4 seconds while device vibrates.

How do you instruct your patient to use acapella?
answer
-Ensure adjustment dial is set to correct range as identified by clinician -Sit with elbows resting comfortable on table -Place mouthpeice lightly in mouth: be sure u maintain a tight seal, nose clips if necessary -Breathe from diaphragm, taking a larger than normal breath, but not filling lungs to capacity -Hold breath 2-3 seconds -Exhale actively but not forcefully, through device. exhalation should last approximately 3-4 times longer than inhalation -Perform 10-20 PEP breaths as recommended by clinician. Remove mouthpiece or mask and perform 2-3 "Huff" coughs to raise secretions -Repeat steps as prescribed

-Inability and tolerated increased work of breathing -ICP>20 -facial, oral or skull surgery or trauma -esophageal surgery -Untreated pneumothorax -Tympanic membrane rupture or middle ear pathology Epitaxis hemoptysis nausea
