Community-based health care is for people of all ages who need health care assistance at home. Community care services include home support, nursing, physiotherapy and other rehabilitation services. Purpose:

Anishnawbe Health Toronto became recognized and funded as a community health centre. TAIBU Community Health Centre (CHC) is a new, multidisciplinary, non-for-profit, community led organization established in the Malvern neighbourhood of Scarborough, Ontario. Casey House is a specialty hospital with community programming including home care and outreach programs. Founded in 1988, it was the first freestanding HIV/AIDS hospice in Canada. Their commitment is to provide compassionate health care to people living with and affected by HIV/AIDS

Equity> is the being just, impartial, and fair. In health care, equity means making resources, such as services and funds, available to everyone, not just certain groups of people Inequity>Inequity is unfairness; favouritism or bias. In health care, inequity means that certain groups of people have more resources available to them. For example, the rural areas have less access to health care professionals that urban areas. Social justice> means fair & that all persons, irrespective of ethnic origin, gender, possessions, race, religion, are to be treated equally and without prejudice. Accessibility> the ability of Canadians to gain the needed health services at the time

A community assessment is a broad look across agencies, systems, and community members to learn more about the circumstances that a partnership has identified as crucial to its vision. It gathers information from many different individuals and groups in the community; the types of information depend on the partnership's focus and the resources it can draw on to conduct the assessment. Most assessments begin by assembling and comparing information already collected by various individual agencies.

A person's social environment includes their living and working conditions, income level, educational background and the communities they are part of. All these have a powerful effect on health. There are big gaps in life expectancy and disease rates between rich and poor, the well and the poorly educated and manual workers and professionals.

Early childhood is the most intensive period of brain development during the lifespan. Adequate stimulation and nutrition are essential for development during the first three years of life. It is during these years that a child's brain is most sensitive to the influences of the external environment. Rapid brain development affects cognitive, social and emotional growth. Such development helps to ensure that each child reaches his or her potential and is a productive part of a rapidly changing, global society. When children spend their early years in a less stimulating, or less emotionally and physically supportive environment, brain development is affected and leads to cognitive, social and behavioral delays

Genetics are also part of the system based on which the health of the population can be established. Genetics are closely related to the habits and behaviors individuals develop during their life, particularly in terms of lifestyle choices. For instance, people who come from families whose members had a more active lifestyle and followed healthier diets, non-smoking and non-drinking are more likely to follow the same pattern in their life. The example set by the family as well as the relationship with friends and family can have a great impact on one's general health. Nonetheless, genetics may play a role in the manner in which people cope with stress.

Individual capacity and coping skills include psychological characteristics such as personal competence and a sense of control and mastery over one's life. These characteristics play an important role in supporting mental and physical health — influencing people's vulnerability to such health problems as cancer and cardiovascular disease, and affecting their risk of unintentional injuries, mental disorders and suicide.
