race & socioeconomic status - lower socioeconomic - hispanic, native american, AA, caucasian Sexual and repro hx -HPV 16,18,31,33 - immunosuppression - smoking

80% squamous cell 15% adenocarcinoma 3-5 adenosquamous

- abnormal vaginal bleeding - postcoital bleeding - watery, mucoid, purulent, malodorous vaginal discharge - pelvic or low back pain - bowel or urinary symptoms

H&P Pap smear, colposcopy, cone of cervix Thorough exam of vagina, rectum and vulva Rectovaginal exam Nodes

Direct extension - parametrial extension - vaginal extension - bladder and rectal involvment Lymphatic extension Hematogenous spread

clinically staged - the stage is set and then never changed even in the face of recurrence Stage 1- confined to the cervix Stage 2 - vagina and parametrium Stage 3 - vaginal, parametrium, hydronephrosis Stage 4 - bladder, rectum, distant mets

- allows resection of bulky lymph nodes - allows assessment of extent of disease for treatment planning
